Checklist for filing a DBA

Check the name availability

You should research the availability of your business name through the County Clerk in the counties in which you will do business and file your assumed name. Many counties now make their records available online.

NOTE- This checklist is for filing with a county. For an explanation of the difference between county and state filing, contact the SBDC (information at the bottom of the page).

Fill out the assumed name form

Free Forms are available at the SBDC office and the County Clerk office.

Sign the form in front of a notary

Only the original, notarized form can be filed with the County Clerk.

Submit the notarized form

Submit the notarized form along with the filing fee to the County Clerk for filing. You can do this in person or by mail. The original will be returned to you by mail for safekeeping.
